Spontaneous Small Intestine Perforation: A Case Report

ÖZET
Spontane ince bağırsak perforasyonu nadir görülen, tedavisi acil cerrahi olan bir akut karın nedenidir. Hastalığın kronik izlem gerektiren hastalıklarla olan birlikteliği, tanısının konmasında gecikmeye neden olmaktadır. Olgumuz 81 yaşında erkek hastadır. Olguda akut böbrek yetmezliği ile hospitalize edilen hastanın bağırsak perforasyonu, klinik seyrin gizlenmesi neticesinde atlanmıştır. Tedavisinde gecikme yaşanmıştır. Hastada iyileşme gözlemlenmemesi ve kliniğin şiddetlenmesi ile yapılan tetkiklerde perforasyon bulguları tespit edildi ve ameliyata alındı. İnce bağırsak Treitz ligamanından itibaren 120. cm’de perfore alan izlendi. Perfore alan eksize edilerek uç ileostomi açıldı. Tedavisi tamamlanan hasta ayaktan takip edilmek üzere taburcu edildi.

ABSTRACT
Spontaneous small intestine perforation is a rare form of acute abdomen that requires emergency surgical treatment. The coexistence of the disease with other diseases that require chronic follow-up leads to a delay in diagnosis. Our acute case is an 81 years old male patient. In this case, the intestine perforation of the patient who was hospitalized with acute renal failure was missed as a result of concealing the clinical course. His treatment was delayed. Perforation findings were detected in the examinations performed due to the patient's lack of improvement and the exacerbation of the clinic, and he underwent surgery. A perforated area was detected at the 120th cm from the small intestine Treitz ligament. The perforated area was excised and the end ileostomy was opened. The patient whose treatment was completed was discharged with outpatient follow-up.
