ÖZET

Künt batın travmaları sırasında karaciğer ve dalak gibi solid organlarla karşılaştırıldığında kolon ve ince barsak yaralanmaları çok daha nadir görülmektedir. Travma sonrası çekum nekrozu, yüksek morbidite ve mortalite ile ilişkili, az tanınan bir antitedir. Tanı koymak, uygun ve zamanında tedaviye başlamak, sonuçların iyileştirilmesi için kritik öneme sahiptir. Çekum nekrozunun patogenezi, oklüziv veya non-oklüziv gibi nedenlerle ilişkilidir. Erken tanı ve nekroze segmentin mümkün olduğunca erken rezeksiyonu yüksek mortalite oranlarını düşürebilir. Travma ile başvuran geçmişinde vasküler hastalık öyküsü bulunan hastalarının ayırıcı tanısında çekum nekrozu da akla gelmelidir. Burada daha önce koroner bypass öyküsü olan trafik kazasına bağlı künt travma sonrası laparotomide çekumda izole çekum nekroz görülen 75 yaşında bir erkek hasta sunuldu.

ABSTRACT

Colon and small intestine injuries are much less common during blunt abdominal trauma compared to solid organs such as the liver and spleen. Post-traumatic cecum necrosis is a less recognized entity associated with high morbidity and mortality. Making a diagnosis, initiating appropriate and timely treatment is critical for improving outcomes. The pathogenesis of cecum necrosis may be associated with occlusive or non-occlusive causes. Early diagnosis and resection of the necrotic segment as early as possible may decrease the high mortality rates. In the differential diagnosis of patients with a history of vascular disease presenting with trauma, cecum necrosis should also be considered. Here, we present a 75 years-old male patient with a history of coronary bypass who underwent laparotomy after blunt trauma due to car accident and who underwent segmental resection end-lateral anastomosis with the diagnosis of isolated necrosis in the cecum.
