Comparison of Commonly Examined Pathologies, COVID-19 Pre-Pandemic and Pandemic Period Data, single-center study

ÖZET
Koronavirüs hastalığı 19 (COVID-19) salgını döneminde sağlık hizmeti sunumunda ciddi zorluklar ortaya çıkardı. Birçok sektörün iş azaltmaya gittiği dönemde sağlık sektöründe çalışma düzeninde modifikasyonlara gidilmeye çalışıldı. Ulusal bazda acil durum öncelikli tedaviler ön plana alınırken farklı tetkik ve tedaviler geri planda kaldı.Polikliniklerde sık karşılaşılan meme, tiroit, mide ve kolorektal hastalıkların tanı ve tedavisinde patolojik verilerin yeri büyüktür. Çalışmamızda pandemi dönemi ile pandemi öncesi dönemde pandemi merkezi olarak çalışmayan ileri basamak bir hastanenin patolojik verilerindeki değişiklik incelenmiştir.Pandemi döneminde 9 aylık periyotta tiroit, meme, mide ve kolorektal alanda 1365 patoloji hizmeti veren merkez, bir önceki yıl aynı dönemde 2551 patoloji hizmeti verdiği görülmüştür. Bu düşüş incelendiğinde tiroit, mide ve kolorektal hastalıklarda tanısal işlemler olan endoskopi, kolonoskopi ve tanısal biyopsilerde ciddi azalmaların olduğu görüldü. Ancak meme biyopsilerinde azalma gözlenmedi. Tiroit ve kolorektal cerrahisi sonucu alınan patolojilerde azalma izlenirken meme ve mide cerrahilerinden elde edilen patolojilerde azalma gözlenmedi.COVID-19 salgını sağlık sisteminde ciddi sorunlar meydana çıkarmıştır. Bunlardan en önemlisi acil olarak nitelendirilmeyen sağlık hizmetlerinde ciddi azalmalar ve ötelemeler yapılmaktadır. Bu yaşananlar ilerde gecikmiş vakalara neden olarak sağlık sistemine ayrı bir yük getirecektir.

ABSTRACT
The coronavirus disease 19 (COVID-19) pandemic has posed serious challenges in healthcare delivery. In the period when many sectors went to reduce jobs, modifications were tried to be made in the working order in the health sector. On a national basis, priority treatments were given priority, while different examinations and treatments remained in the background. Pathological data have a great place in the diagnosis and treatment of breast, thyroid, stomach, and colorectal diseases that are frequently encountered in outpatient clinics. In our study, the changes in the pathological data of an advanced hospital that did not work as a pandemic center during the pandemic period and pre-pandemic period were examined. It was observed that the centers providing 1365 pathology services in the thyroid, breast, stomach, and colorectal areas in 9 months during the pandemic period, provided 2551 pathology services in the same period of the previous year. When this decrease was examined, it was observed that there were serious decreases in endoscopy, colonoscopy, and diagnostic biopsies, which are diagnostic procedures in thyroid, stomach, and colorectal diseases. However, no reduction was observed in breast biopsies. While a decrease was observed in pathologies obtained as a result of thyroid and colorectal surgery, no decrease was observed in pathologies obtained from breast and stomach surgeries. The COVID-19 outbreak has created serious problems in the health system. The most important of these are serious reductions and postponements in health services that are not considered urgent. These experiences will cause delayed cases in the future and will place a separate burden on the health system.
